Proven Solutions to Address Social Determinants of Health

Sellers Dorsey is pleased to share that Gary Jessee, Managing Director, will be moderating a panel discussion at the ADvancing States’ Home- and Community-Based Services (HCBS) Conference. The session, Proven Strategies to Address Social Determinants of Health, will be on August 17 at 1:15pm–2:15pm ET in Washington, DC.

Social determinants of health (SDOH) are conditions in the environment that affect a range of health, functioning, and outcomes. Addressing issues of housing, nutrition, and financial independence, specifically, can have a significant impact on one’s overall health.

To address these challenges, our panelists are offering collaborative and innovative solutions to tackle disparities and inequities while promoting independence.

  • The Iowa Department of Aging has implemented a pilot program called The Iowa Café providing options for older adults to eat at local restaurants utilizing a streamlined check-in system. The program, funded by emergency funds and Older Americans Act Title IIIC funds, addresses nutrition and social isolation, allows freedom of choice, and promotes independence for older adults.
  • Losing capacity to maintain bills and budgets (an instrumental activity of daily living) can lead to exploitation and reduce an individual’s independence. It’s also a significant factor affecting guardianship and housing decisions. SilverBills offers concierge bill-pay programs for older adults and caregivers by managing the entire bill paying process, reducing the stress of money management while promoting independence and safety.
  • As the critical link between access to safe, affordable housing and an individual’s health and independence becomes clearer, health plans like United Healthcare are taking the lead in developing innovative solutions focused on integrating housing and health care by investing in affordable housing options for vulnerable populations.
