Improving Healthcare Quality and Access Throughout Kentucky

Share:

Project Summary

Sellers Dorsey worked with a long-standing provider coalition, including the University of Louisville School of Medicine, UofL Health (collectively UofL), and the Norton Children’s Medical Group (NCMG), to implement a directed payment program (DPP) that improved quality, access, and outcomes for patients across Kentucky.

One of the most prevalent health systems in the state, and the area’s largest pediatric provider, UofL Health and NCMG, are foundational to supporting the health and well-being of Kentucky residents. By redesigning Medicaid managed care programs, increasing provider resources, and raising funding levels, the DPP helped improve quality of care for Medicaid members and expand access to value-based care.

Program Focus

This program is critical to the health of Kentucky’s future generations as it provides both primary care and pediatric specialty care to one in five children in the Kentucky Medicaid program. This initiative supports statewide access to UofL’s healthcare system, comprising 9 hospitals, more than 250 Ambulatory clinics, including 50 pediatric clinics, and a faculty practice with more than 80 subspecialties.

Sellers Dorsey has worked with the coalition since 2018 to:

  • Preserve and enhance the existing supplemental funding stream of UofL.
  • Achieve double-digit improvement on many of the program’s 21 quality measures.
  • Qualify providers for 100% of the at-risk quality performance-based payments.

Program Creation

Sellers Dorsey worked closely with the coalition to:

  • Design a transformative, value-based program.
  • Educate key leaders on innovative methodologies and approaches.
  • Increase the hospital payment methodology to the commercial equivalent for all coalition members.
  • Develop relationships with key coalition stakeholders, State officials, and vendors.
  • Secure annual CMS approval through 2026 with continued support into the future
  • Facilitate implementation and provider performance.

“I’m so pleased with all the success we have enjoyed over the past few years and know we would not be where we are without the help of Sellers Dorsey. I am very appreciative of that help and the relationship we have developed.”
– Jerry Johnson, Chief of Staff to the Executive Vice President for Health Affairs, University of Louisville

Emerging Impacts

Impacts from the first six years of program funding include:

  • Secured the ongoing operation of a critical Louisville hospital, which is now recognized as “High Performing” by S. News & World Report in four specialty conditions and procedures, including heart and lung care.
  • Expanded access to care in underserved communities by establishing an urgent care clinic in a medically underserved neighborhood.
  • Advanced “total person care” delivery through integrated depression and social drivers of health screenings, alongside embedded behavioral health services in pediatric primary care.
  • Achieved top-tier pediatric preventive care performance, surpassing the national 90th percentile for well-child visits in the first 15 months of life, representing a 75% increase since program inception.
  • Improved cancer prevention and early detection, with breast and colorectal screening rates significantly exceeding statewide benchmarks, contributing to improved survival outcomes.
  • Built and operationalized actionable analytics capabilities, enabling data-driven workflow redesigns and measurable improvements in care delivery and patient outcomes.
